The structure of the protein will be affected if the protein is hydrophillic in nature while the R group is hydrophobic in nature.

The tertiary structure of a protein is determined by the interaction and bond of the R group within a protein. Thus, if a mutation occur at the R group side chain the tertiary structure of the protein will be affected.
