The nurse must delegate the UAP to give the patient a bath depending on what kind of sore the patient has. Considering that it is only a mild breakdown, mild soap and warm water will do. If there is a recommended cleanser, then use it. It is also important to keep the bath warm for the patient and to avoid water from spraying on the patient's face.
