While walking in a hallway, a client with a chest tube becomes confused and pulls the chest tube out. what is the nurse's immediate action?

    The nurse should be smart and alert with his actions. He must approach the patient at once without panicking the patient. He approach with great understanding and gentle interruption on what the patient is about to do. This is to prevent further fear or nervousness from the patient that may lead to even severe accidents.
