An atom is a neutral species. Hence, it must have same number of electrons and protons. On other hand, ions have charge. Hence, number of protons and electrons are not same.

In present case, hydrogen has one proton, two neutrons, and no electrons. This signifies that overall charge of species is + 1. Hence, it is an ion.

Yes this is an ion because an ion is defined as an element that has gained or lost electrons. There should be 1 electron is Hydrogen but since there aren't any that means that it is an ion which is commonly written as H^+.
