How can miscommunication lead to tragedy

Let's say we're in Revolutionary times. A solider overhears a Red Coats plans to attack a certain fort. If that solider were to tell their General he saw red coats, and they're attacking a fort, they wouldn't know what fort because the solider didn't communicate the plans he heard correctly. Now, they don't know what fort is going to be attacked.
