    In y = mx + b form, the slope will be in the m position (the number right before x) and the y intercept will be in the b position

    y = mx + b

    slope (m) = - 4

    y int (b) = 5

    now just sub

    y = - 4x + 5 < = = = or if u need it in standard form : 4x + y = 5
