    You do not specify the country. I suspect it might be the United States. In case it is Canada, it is form 2221 and lines 330 and 331.

    In the United States it is Form 1040 Schedule A but you better read document 502 listing what you can and cannot deduct.

    One thing is for absolute certain: if there is any kind of wrinkle in your taxes, do not fill them out yourself. Get an expert to do it. It is worth the extra money not to get yourself in hot water with the IRS or the CRA.
