You must complete a 4 year degree in biology, anatomy/phy, pre-medicine etc. Then you must have 4 years in medical school. After that you must complete a 4-5 year surgical internship/residency at the hospital. So in total its 12 + years to be at the top. but during your 4-5 year surgical residency you will be able to scub in / assist on surgery.
