I'm not sure if I'm correct but the first thing I'd do is report my finding to the Child Protective Services and the police.

The Child Protective Services will know what to do in this type of scenarios because they are tasked to provide protection to a child that have been abused or neglected.

The police will also know what to do in the event that the parents knew about the abuse and did not report it.

Telling the parents that their child has been abused is the last thing to do because it may put the child in more danger. What if the parents were the ones who initiated the abuse or someone they trusted did the horrible deed and upon knowledge of the fact, they will either hide evidence or find the person responsible and assault said person. Both scenarios will not benefit the child.
