    I believe the answer is: Software failure followed by human error

    Various researchers indicated that around 70% of downtime exist because of human errors. The software failure is usually could be anticipated and back up plan usually already established for such situation. But, combined with human error, no fast course of action is provided to address the failure, leading up to longer period of down time.
