The codes for the two stages are 17311, 17312, and 17315.

Mohs codes are being selected based on location and also the number of stages for each including the five blocks. The add-on code for each additional block after the first five blocks in the five stages.

Code 17311 is for the first stage which has four tissue blocks, code 17312 which is for the second stage has five tissue blocks which are documentation of the site forehead.

The other six tissues which block are prepared in the second stage and they are reported with add-on code which is 17315.
