    y=-1/5x-1/5

    I assume you were saying that the y-int = - 1/5 and the slope = - 1/5. If so, here we go.

    Slope intercept form is y=mx+b

    y = y

    m = slope

    x = x

    b = y-intercept

    from here on you just plug in numbers

    y = (-1/5) x + (-1/5)

    Since b is a negative, it changes to subtraction because you don't add negatives

    so the answer would be

    y = - 1/5x-1/5
