The information is in AIM 5-1-1 (f):

    Explanation:

If you request a briefing, describe yourself as a pilot and give these information:

type of flight scheduled; for instance, VFR or IFR.

Number of the plane or name of the pilot.

    Type of aircraft.

    check-out airport.

    The flight path.

    The Plane altitude.

    ETE & ETD

Example like: This is J35670, I would love a standard briefing for a IFR flight from DKEB to SNEV at 2200', ETD at 1200Z, ETE 3h19
