Which of the following is a question you should ask your boss regarding individual performance evaluations?

I guess I should ask if I have improved as an employee and if I have contributed more to the office. Apart from declining performance, it is also bad if there is no improvement in the way an employee does things at work. Others factors an employee should consider is if he is growing in the company and if he is getting along with co-workers.
