Who will eventually have to pay real money for your mistake if you kill someone when driving while impaired?

    It would be yourself.

    If you killed someone when you're drunk first you have to pay the fine to the government, and that's not the expensive one.

    The expensive payment would be for the property damage that you cause and the lawsuits that the victim's family potentially give to you.
